29 CFR 1910.215(b)(9): Exposure adjustment. The distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceed one-fourth inch. A) In the Casino's general shop there was a bench grinder with the tongue guards installed backward and, as a result, were not adjusted to within one-fourth inch of the surface of the grinding wheel. When operating the grinder, the operator stands in front of the wheel and the tongue guard is designed to stop any flying objects from hitting the operator. Because the tongue guards were not properly installed and adjusted, employees were exposed to contact with any parts of the wheel and being struck by an item being ground.

29 CFR 1910.305(b)(2)(i): Each outlet box did not have a cover, faceplate, or fixture canopy. A) Inside the general shop, one of the wall mounted electrical outlets did not have a face cover installed. An open electrical box exposes employees to potential electric shock. There is also a danger that contaminants could enter the box and interfere with the electrical components and potentially start a fire.
