29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a) 125 Bennet Avenue, Bldg# 90710 (SNC Warehouse), Hurlburt Field, FL: On or about April 4, 2022, and at times prior thereto, the controlling employer U.S. Department of the Air Force failed to ensure that its subcontractor Sierra Nevada Corporation protected its employees from unsafe practices in that employees were allowed to utilize a forklift fork extension attachment without written approval from the forklift manufacturer (or qualified Registered Professional Engineer) for the attachment to be used with the forklift.
