On or about 05/02/2022, at 1820 Gulf Shore Blvd, Naples, Florida, the employer failed to ensure that the company safety program was enforced by allowing employees to be exposed to hazards such as, falls from lack of guard rails, defective planking on scaffolding, no hard hats, lack of toeboards, while employees were standing on concrete blocks on platforms and single plank used as a working platform.

29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units: a) On or about 05/02/2022, at 1820 Gulfshore Blvd. North, Naples, Florida, employees were applying concrete stucco to the exterior walls of a building under construction were standing on a plank that was supported by a concrete block on each end of the plank that was placed on the scaffold planking were exposed to a fall hazard. b) On or about 05/02/2022, at 1820 Gulfshore Blvd. North, Naples, Florida, employees were working from multiple levels of scaffolding applying concrete stucco to the exterior walls of a building under construction where two single planks were used as platforms on the second and third levels of the scaffold were exposed to a fall hazard.
