29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about May 9, 2022, at the above addressed jobsite, an employee using a handheld circular saw was not wearing safety glasses, thereby exposing employee to hazards associated with eye injuries.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about May 9, 2020, at the above addressed jobsite, an employee performing residential carpentry operations, eleven (11) feet three (3) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.
