On or about December 11, 2024, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

On or about December 11, 2024, at the above addressed jobsite, an employee using a handheld circular saw was not wearing safety glasses, thereby exposing employee to hazards associated with eye injuries.  b. On or about December 11, 2024, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing employee to hazards associated with eye injuries.
