On or about September 8, 2022, at the above addressed jobsite, an employee, performing window installation operations from a platform raised, approximately fifteen (15) feet above the ground, by the forks of a power industrial vehicle/OmniQuip FL8Q22 forklift, did not have the platform attached to the forks, thereby exposing the employee to fall hazards.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: a. On or about September 8, 2022, at the above addressed jobsite, the employer was using an unapproved attachment (wooden job made box) on the forks of a power industrial vehicle/OmniQuip FL8Q22 forklift to hoist an employee to the roof of a new residential construction. This practice could affect the capacity of the equipment and the manufacturer's written approval was not obtained, thereby exposing the employees to a struck by hazards due to forklift tip over.
