29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about May 10, 2022 an employee operating the TruBend 5230 Hydraulic Press Brake was exposed to amputation hazards created by pinch points between metal parts and the tooling, dies, or upper ram. An employee's right middle finger tip was amputated when it was pinched between a metal part raised up and made contact with the upper ram. The employer provided no means of guarding to prevent access to the pinch point.
