29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded or guards were not designed and constructed to prevent employees from having any part of their body in the danger zones during operating cycles: (a)Twin City Fan Companies, Ltd. at 101 Sunrise Ridge Road South, Brookings, SD 57006: On or about and at times prior to May 24, 2022, the employer did not ensure the point of operation on the Lapointe Broaching Machine, Serial Number: R124 was properly guarded to prevent employees from having any part of the their body in the danger zone during machine operations. This hazard exposed employees to crushing and amputation hazards. (b)Twin City Fan Companies, Ltd. at 101 Sunrise Ridge Road South, Brookings, SD 57006: On or about and at times prior to May 24, 2022, the employer did not ensure the point of operation on the Uni-Hydro Incorporated Ironworker, Model Number: 56-14, Serial Number: 3PE7009X was properly guarded to prevent employees from having any part of the their body in the danger zone during machine operations. This hazard exposed employees to crushing and amputation hazards. (c)Twin City Fan Companies, Ltd. at 101 Sunrise Ridge Road South, Brookings, SD 57006: On or about and at times prior to May 24, 2022, the employer did not ensure the points of operation on the Piranha Ironworker, Serial Number: P120 1062 was properly guarded to prevent employees from having any part of the their body in the danger zones during machine operations. This hazard exposed employees to crushing and amputation hazards.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: (a) Twin City Fan Companies, Ltd. at 101 Sunrise Ridge Road South, Brookings, SD 57006: On or about and at times prior to May 24, 2022, the employer did not ensure that electrical equipment was effectively closed in that employees were exposed to live electrical circuits associated with one open 220-volt electrical junction box in the first welding booth located in the southwest manual welding area . This condition exposed employees to shock and electrocution as well as fire hazards associated with exposed wiring on the electrical box.
