29 CFR 1910.212(a)(3)(ii): Point of operation of machinery were not guarded or guards were not designed and constructed to prevent employees from having any part of their body in the danger zones during operating cycles: (a)Twin City Fan Companies, Ltd. at 500 Insbrook Lane, Brookings, SD 57006: On or about and at times prior to May 25, 2022, the employer did not ensure the point of operation on the Huck Hydraulic Powerig Press, Model: 908, Serial number: B2WH-0009 was properly guarded to prevent employees from having any part of the their body in the danger zone during machine operations. This condition exposed employees to crushing and amputation hazards.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: (a) Twin City Fan Companies, Ltd. at 500 Insbrook Lane, Brookings, SD 57006: On or about and at times prior to May 25, 2022, the employer did not ensure that electrical equipment was effectively closed in that employees were exposed to live electrical circuits associated with one open 30 amp/480-volt electrical junction box in a welding area located along the west wall of the wheel weld area . This condition exposed employees to shock and electrocution as well as fire hazards associated with exposed wiring on the electrical box.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: (a) Twin City Fan Companies, Ltd. at 500 Insbrook Lane, Brookings, SD 57006: On or about and at times prior to May 25, 2022, the employer did not ensure the electrical extension cord being utilized on the west wall of the fabrication and wheel welding area was not used as permanent wiring for Dayton 20-inch Drill Press, Model: 49G986A, Serial number: 83U0969. This condition exposed employees to electrical shock hazards associated with improper use of electrical equipment.
