(a) On or about May 31, 2022 at the job site located at 6110 West Bluemound Rd, Wauwatosa WI, an employee was performing roofing work on a two story residential building with pneumatic nail guns without safety glasses.

The Kike's Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.501(b)(11), which was contained in OSHA inspection number 1384312, citation number 1, item number 1a and was affirmed as a final order on April 16, 2019, with respect to a workplace located at 9592 Kettle Moraine Drive, Kewaskum WI 53040.
