29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about June 25, 2024, at the jobsite located at 2250 Belmont Pl, Oak Creek WI 53154, employees cleaning up roofing material on the ground directly below other employees conducting roofing work above them did not have head protection, exposing them to struck by hazards.  The Kike's Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.100(a), which was contained in OSHA inspection number 1599709, citation number 1, item number 1 and was affirmed as a final order on November 15, 2022, with respect to a workplace located at 6110 West Bluemound Rd, Wauwatosa, WI 53213.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about June 25, 2024, at the jobsite located at 2250 Belmont Pl, Oak Creek WI 53154, the employer did not ensure employees installing shingles on a two-story residential home using a pneumatic nail gun used eye protection, exposing the employees to eye hazards from flying particles.  The Kike's Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.102(a)(1), which was contained in OSHA inspection number 1611057, citation number 1, item number 1 and was affirmed as a final order on December 7, 2022, with respect to a workplace located at 1360 Bluebird Drive, Oconomowoc, WI 53066.  The Kike's Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.102(a)(1), which was contained in OSHA inspection number 1599709, citation number 1, item number 2 and was affirmed as a final order on November 15, 2022, with respect to a workplace located at 6110 West Bluemound Rd, Wauwatosa, WI 53213.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about June 25, 2024, at the jobsite located at 2250 Belmont Pl, Oak Creek WI 53154, employees installing new shingles were not using fall protection. Six employees had fall protection harness on but were not tied off and four employees did not have harnesses on. This exposed employees to fall hazard of approximately 15 3/10 feet.  The Kike's Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.501(b)(11), which was contained in OSHA inspection number 1611057, citation number 1, item number 2 and was affirmed as a final order on December 7, 2022, with respect to a workplace located at 1360 Bluebird Drive, Oconomowoc, WI 53066. 29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  (a) On or about June 25, 2024, at the jobsite located at 2250 Belmont Pl, Oak Creek WI 53154, the employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with performing roofing work on a residential home.  The Kike's Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.503(a)(1), which was contained in OSHA inspection number 1599709, citation number 1, item number 3b and was affirmed as a final order on November 15, 2022, with respect to a workplace located at 6110 West Bluemound Road, Wauwatosa, WI 53213
