29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about August 24, 2023, at the jobsite located at 443 Landimore Lane, Wales, WI 53183, employees cleaning up roofing material on the ground directly below other employees conducting roofing work above them did not have head protection, exposing them to struck by hazards.  The Kike's Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.100(a), which was contained in OSHA inspection number 1599709, citation number 1, item number 1 and was affirmed as a final order on November 15, 2022, with respect to a workplace located at 6110 West Bluemound Rd, Wauwatosa, WI 53213.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about August 24, 2023, at the jobsite located at 443 Landimore Lane, Wales, WI 53183, the employer did not ensure employees installing shingles on a two-story residential home using a pneumatic nail gun used eye protection, exposing the employee to eye hazards from flying particles.  The Kike's Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.102(a)(1), which was contained in OSHA inspection number 1599709, citation number 1, item number 2 and was affirmed as a final order on November 15, 2022, with respect to a workplace located at 6110 West Bluemound Rd, Wauwatosa, WI 53213.  The Kike's Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.102(a)(1), which was contained in OSHA inspection number 1611057, citation number 1, item number 1 and was affirmed as a final order on December 7, 2022, with respect to a workplace located at 1360 Bluebird Drive, Oconomowoc, WI 53066.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  (a) On or about August 24, 2023, at the jobsite located at 443 Landimore Lane, Wales, WI 53183, the employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with performing roofing work on a residential home.
