29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  The employer does not protect each employee from struck-by hazards such as potential eye or face injury from physical, chemical, or radiation agents while installing roofing material with air driven nail guns. This most recently occurred on June 3, 2022, during the installation of roofing material at a job site located at Lot 71 Reserve Dr, Tallmadge, Ohio 44278; the employer did not ensure employees working on the roof of a single-family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material with air driven nail guns.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   The employer does not protect each employee from fall hazards while performing roofing activities on residential structures with unprotected sides and edges at height of approximately 22 feet above the ground by the use of guard rail systems, safety net systems, and or personal fall arrest systems. This most recently occurred on June 3, 2022, during the installation of roofing material at a job site located at Lot 71 Reserve Dr, Tallmadge, Ohio 44278; the employer did not ensure that all employees working on the roof of a single-family home were protected from a fall of approximately 22 feet by utilizing a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 3, 2022, at the job site located at Lot 71 Reserve Dr, Tallmadge, Ohio 44278; during the installation of roofing material on a residential roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface.  C.R.H. Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.1053(B)(1) Access/Egress, which was contained in OSHA inspection number 1570528, citation number 1, item number 2, and was affirmed as a final order on 01/31/2022, with respect to a workplace located at Lot 27 Emerald Woods, Columbia Station, Ohio 44028.
