29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end. a- Back of Structure, 33 Westmont Drive, Medford NJ - A worker was exposed to fall hazards, up to and in excess of 14 feet 2 inches to the ground surface below, when performing siding installation and other related activities from a pump jack scaffold whose unsecured walking and working surface extended up to and in excess of 5 feet beyond its support/pole. Condition observed on or about 06/15/22. b- Side of Structure, 33 Westmont Drive, Medford NJ - A worker was exposed to fall hazards, up to and in excess of 14 feet 2 inches to the ground surface below, when performing siding installation and other related activities from a pump jack scaffold whose unsecured walking and working surface extended in excess of 4 and a half feet beyond both of its supports/poles. Condition observed on or about 06/15/22. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a- 33 Westmont Drive, Medford NJ - Two workers were exposed to fall hazards, up to and in excess of 14 feet 2 inches to the ground surface below, when performing siding installation and other related activities from two separate pump jack scaffolds that both did not contain a screen that ran the full length of their active walking and working surface and were both missing end stops. Condition observed on or about 06/15/22. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
