Roof area located at 1317 W 30th Street, Jacksonville, Florida 32209: On or about June 28, 2022, the employer exposed employees to eye injuries caused by airborne debris and flying objects, in that eye protection was not utilized by employee, while using a pneumatic nail gun to shingle the roof.
