29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Jobsite located at 250 North Bradford Avenue, West Chester, PA.: The employer did not ensure that employees utilized protective helmets at the jobsite while an unsecured load was suspended over the employees head, thus exposing them to struck-by hazards.  Condition observed on or about June 30, 2022.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system.    a) Jobsite located at 250 North Bradford Avenue, West Chester, PA.: The employer did not ensure that employees conducting residential framing operations on the 4th story of the building, approximately 30 feet from the ground were protected from fall hazards by the use of a fall protection system.  Condition was observed on or about June 30, 2022

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite located at 250 North Bradford Avenue, West Chester, PA.: The employer did not provide a training program for each employee conducting residential framing activities in the recognition and avoidance of fall hazards. Employees were working in close proximity to the unprotected sides and edges of the 4th floor of a residential building and exposed to falls of approximately 30 feet.  Condition was observed on or about June 30, 2022.
