29 CFR  1926.302(b)(6): Air hoses were used for hoisting and lowering of tools.   a- 36 Staton Road, Mount Holly NJ - On or about 11/08/22 a worker lowered an air nailer, from an interior scaffold to the floor below, using the air hose it was connected to.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR  1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.   a - 32 Staton Road, Mount Holly NJ: Workers were exposed to fall hazards, in excess of 12 feet 9 inches to the ground surface, when accessing and egressing an A-framed ladder that was set outside of an unprotected window opening having an inside bottom edge approximately 24 inches above the walking/working surface. Condition noted  on or about 11/08/22.  b - 36 Staton Road, Mount Holly NJ: A worker was exposed to fall hazards, in excess of 12 feet 6 inches to the ground surface, when retrieving 2 x 4s from an unprotected window opening having an inside bottom edge approximately 24 inches above the walking/working surface.  Condition noted  on or about 11/08/22.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.   a- 32, 34, 36 Staton Road, Mount Holly NJ:  Workers were exposed to fall hazards, up to and in excess of 11 feet to the ground  surface below, when using a the top step of an A-framed ladder and a damaged front section of an A-framed ladder (was missing its back section) at the worksite. These workers did not receive ladder hazard recognition and minimization training from the employer at the time of the worksite inspection.  Condition observed on or about 11/08/22.

29 CFR  1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)    a- 32, 34, 36 Staton Road, Mount Holly NJ:  Workers, not utilizing fall protection, were exposed to fall hazards in excess of 9 feet 5 inches to surfaces below when performing roof truss installation, framing, and other related activities while not utilizing or supplied with fall protection at the worksite.   These workers had not received fall protection training as specified in 29 CFR 1926.503 (a)(2)(i) through (viii).  Condition noted on or about 11/08/22.  Green Master Contractor Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(2), which was contained in OSHA inspection number 1589276, citation number 1, item number 4 and was affirmed as a final order on 09/21/22, with respect to a workplace located at Foundry Boulevard, Mount Holly NJ.
