Bowling Green OH 43402, did not ensure employees were protected from falls while conducting residential roofing construction activities on the second (roof) level of a residential single family home, while exposed to a fall of approximately 19 feet or greater to the ground below.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about July 7, 2022 the employer Joaquin Tovar Perez at jobsite located at 1018 Boone Ct. Bowling Green OH 43402, did not ensure the side rails of one portable aluminum extension ladder was extended at least three feet above the landing surface. Six employees were exposed to falls from approximately 9 feet or greater from the top of the second (roof) level to the decking below while using the ladder to access the second (roof) level to perform residential roofing activities on a single family home.
