Weld Shop: On and/or before July 7, 2022, an orange Werner 8 foot A-frame ladder was used for maintenance to access elevated positions of 10 to 12 feet. The ladder had an anti-slip safety shoe structural defect. Employee(s) were exposed to fall hazards.

Weld Shop: On and/or before July 7, 2022, an orange Werner 8 foot A-frame ladder was used for maintenance to access elevated positions of 10 to 12 feet.
