29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects was immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d), or replaced.  Weld Shop: On and/or before July 7, 2022, an orange Werner 8 foot A-frame ladder was used for maintenance to access elevated positions of 10 to 12 feet. The ladder had an anti-slip safety shoe structural defect. Employee(s) were exposed to fall hazards.

Weld Shop: On and/or before July 7, 2022, an orange Werner 8 foot A-frame ladder was used for maintenance to access elevated positions of 10 to 12 feet. The A frame ladder used did not have any manufacture data listed for load limits for employees to identify during inspection and use of ladder. Employees were exposed to fall hazard(s).
