29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a)  On or about July 13, 2022, employees were using an extension ladder which did not extend 3 feet above the upper landing being accessed.   b) On or about July 13, 2022, employees were using a section of an extension ladder which did not extend 3 feet above the upper landing being accessed.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)  On July 13, 2022, employees working below roofing operations where debris was being thrown were exposed to struck-by hazards were not wearing hard hats.  AG General Construction Services PLLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a) which was contained in OSHA inspection number 1061825, citation 1, item 1, and was affirmed final order on June 21, 2021, with respect to a workplace located at 501 Kenilworth Ave, Kenilworth, IL 60043.  No abatement certification or documentation is required for this item.

AG General Construction Services PLLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1061825, citation 1, item 2, and was affirmed final order on June 21, 2021, with respect to a workplace located at 501 Kenilworth Ave, Kenilworth, IL 60043.  No abatement certification or documentation is required for this item.
