29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:  a) On October 18, 2024, at the above addressed jobsite, employees were exposed to eye injury while operating pneumatic nail guns without eye protection.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On October 18, 2024, at the above addressed jobsite, employees conducting metal work from a ladder jack scaffold, approximately 18 above the ground, were not provided with personal fall arrest system(s), exposing the employees to falls.  No abatement certification or documentation is required for this item.

29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On October 18, 2024, at the above addressed jobsite, employees were exposed to struck-by hazard from falling tools, material and debris when working below employees performing roofing work.  The employees were not wearing hard hats.  AG General Construction Services PLLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1607441, citation number 2, item number 1 and was affirmed as a final order on December 7, 2022, with respect to a workplace located at 739 Ascot Court in Libertyville, Illinois.  AG General Construction Services LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1506182, citation number 1, item number 1 and was affirmed as a final order on June 21, 2021, with respect to a workplace located at 501 Kenilworth Avenue in Kenilworth, Illinois.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
