29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about July 14, 2022, employees working on the lower level of the roof were not wearing protective helmets to protect them from roofing debris being torn off above them.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a)  On or about July 14, 2022, employees conducting re-roofing of a  residential building roof, did not use fall protection. This exposed  employees to a fall hazard of approximately nine feet, eight inch (9.8 feet) fall hazard.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about July 14, 2022, employees working on a residential building, and exposed to fall hazards of approximately nine feet, eight inches (9.8 feet), were not provided with a training program to identify fall hazards and methods to eliminate or minimize those hazards.
