29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about July 21, 2022 on line 3, at the cyclone and SWECO, employees perform servicing and maintenance on equipment including, but not limited to the cyclone. The employer did not develop, document or ensure employees utilized machine specific lockout tagout procedures to prevent exposure to the air valve when unclogging product in the cyclone. Producers Rice Mill, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1436605, citation number 1, item number 1 and was affirmed as a final order on April 20, 2020, with respect to a workplace located at 518 East Harrison Street, Stuttgart, AR 72160.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA From 300 or equivalent), was not completed in detail as required by the regulation: On or about July 21, 2022, at the facility, the employer id not properly describe each recordable injury/illness in the following instances of injuries listed on the 2022 OSHA 300 log: a) On or about June 8, 2022, Case number 4, Extrusion Operator received an injury resulting in days away from work. The log did not include a description of the injury sustained or the body part(s) affected. b) On or about June 13, 2022, Case number 5, Extrusion Operator received a cut to the hand resulting in days away from work. The description on the log did not include which hand was injured. c) On or about July 18, 2022, Case number 6, Extrusion Operator received an injury resulting in amputation of three fingers. The description on the log did not identify the injury as a three-finger amputation.
