29 CFR 1910.36(d)(2): Exit route doors were operated by a device or alarm that could restrict emergency use of the exit route if the device or alarm fails: On or about May 14, 2024, at the parboil mill, employees enter and exit through entry and exit route doors separated by a corridor. The doors did not open as intended when the exit button was pressed.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about May 7, 2024, Parboil Division, in between soaking tanks #9 and #10, employees performed servicing and maintenance on equipment including, but not limited to flex hose valves on soaking tanks. Specific hazardous energy control procedures were not developed, documented or utilized. Producers Rice Mill, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1609512, Citation 2, Item 1 and was affirmed as a final order on August 31, 2023, with respect to a workplace located at 603 North Park Avenue, Stuttgart, AR 72160.
