29 CFR 1910.22(a)(1):29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. (a) At a workplace located at 19886 Lawrence 2145 Aurora, MO ; employees were exposed to fire and combustible dust hazards. The employer failed to provide surfaces that are free of hazards such as accumulation of grass dust. Dust was observed near the hammer mill area on the side wall girts and included, but not limited to, a depth of six inches of grass dust, a combustible dust, on elevated structures and surfaces.

The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided. (a) At a workplace located at 19886 Lawrence 2145 Aurora, MO ; employees were exposed to fire and combustible dust hazards. The employer failed to provide floor surfaces that are free of hazards such as accumulation of grass dust. Dust was observed near the hammer mill area and included, but not limited to, a depth of two feet of grass dust, a combustible dust, on the floor surface.

There were two electrical boxes near the hammer mill area that was not rated for class II division 1 locations. (c) At a workplace located at 19886 Lawrence 2145 Aurora, MO; employees were exposed to fire and combustible dust hazards when working in suspended grass dust particles, a combustible dust. There were two electrical boxes in the production area that were missing faceplates and were not rated for class II division 1 locations.
