29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  At a worksite located at 19886 Lawrence 2145, Aurora, MO 65605, employees were exposed to fire and combustible dust hazards.  The employer exposed the employees to these hazards by failing to provide surfaces that are free of hazards such as the accumulation of paper dust.  Dust was observed near the cooling tower area on the side wall girts and included, but not limited to, a depth of one quarter of an inch of paper dust, a combustible dust, as well as on elevated structures and surfaces.
