29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: At a worksite located at 1306 South Powell Road, Independence, Missouri. Employees were exposed to serious struck by injuries from the Hyster 120 forklift, serial # D0004D02174T, known to the company as forklift 01, that was in operation, and the brake pad was found to be missing, and in need of repair.
