On the FOX bandsaw: On or about March 30, 2023, and at times prior, the employer exposed employees to amputation hazards, in that the blade guard was missing. b. On the Two Foot Mill: On or about March 30, 2023, and at times prior, the employer exposed employees to amputation hazards, in that the guard protecting employees from the chain and sprocket was missing.

29 CFR 1910.219(c)(4)(i):Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: a: On the Forstner Slit line Machine, on or about March 30 and at times prior, the employer is exposing employees to caught in / between and amputation hazards in that there is an protruding shaft end that did not present a smooth edge end and projected out 2 1/4 inches from the end of the 1 inch diameter shaft. The shaft end was not guarded by nonrotating caps or sleeves.
