29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  Jobsite located at 800 Callowhill St, Philadelphia, PA - The employer did not ensure each employee engaged in residential framing activities 6 feet or more above lower levels were t protected by guardrail systems, safety net system, or personal fall arrest system.   a) The employees were framing the second story, adjacent to leading edges, without the use of fall protection, exposing employees to falls approximately 10 feet from the ground.  b) The employees were framing the first and second stories were exposed to fall hazards of approximately 10 feet from the next lower level while working adjacent to unprotected floor openings.    Condition noted on/about July 29, 2022

29 CFR  1926.602(d): The employer did not ensure that each employee operating a powered industrial vehicle had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):  a) Jobsite located at 800 Callowhill St, Philadelphia, PA - On or about July 29, 2022, the employer did not ensure that each employee operating a powered industrial vehicle had been trained and evaluated. An employee was operating a JLG G10-55A forklift and was not trained or evaluated as required by the standard, thereby exposing employees to struck-by hazards.

29 CFR  1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  a) Jobsite located at 800 Callowhill St, Philadelphia, PA - On or about July 29, 2022, the employer did not ensure stairways or ladders were provided at all personnel points of access where there was a break in elevation of 19 inches or more. Employees were using the crossmembers of the structural framing to access the second story, thereby exposing employees to fall hazards of approximately 10 feet.
