29 CFR  1910.134(g)(2)(iii): The employer did not repair or replace the respirator before allowing an employee to return to the work area when the employee detected vapor or gas breakthrough, changes in breathing resistance, or leakage of the facepiece.  1915.154: Respiratory protection for shipyard employment is covered by 29 CFR 1910.134.  M/V Pup: Employees conducted vessel retrofit operations, steel plate removal, steel plate cutting, engine replacement, welding, and painting operations where half mask negative pressure respirators (with cartridge) were required.  The employer did not ensure employees replaced respirators or cartridges when employees detected vapor breakthrough.  Employees potentially exposed to upper respiratory irritation.
