This most recently occurred on and preceding August 12, 2022, at 3631 Sullivant Avenue Columbus, OH 43228. Boxes, bins, cleaning supplies and stacked merchandise are in front of two fire extinguishers in the storeroom obstructing access, which exposes employees to fire hazards. Family Dollar Stores Inc. was previously cited under the name Family Dollar Stores of Texas, LLC for a Repeat-Serious violation of this occupational safety and health standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 1581314, citation number 1, item number 4 and was affirmed as a final order on August 1, 2022, with respect to a workplace located at 2323 FM 1960 West, HOUSTON, TX 77068. Employees are exposed to struck-by hazards where boxed merchandise, stacked above head height, and in excess of 7 feet high, is not arranged in a stable and secure manner to prevent it from falling onto or in the path of employees. Family Dollar Stores, Inc. was previously cited under the name Family Dollar Stores of Rhode Island, LLC for a Repeat-Serious violation of this occupational safety and health standard, 29 CFR 1910.176(b), which was contained in OSHA inspection number 1454287, citation number 1, item number 1 and was affirmed as a final order on June 11, 2020, with respect to a workplace located at 214 Taunton Ave., East Providence, RI 02914.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a) The employer does not ensure that a junction box containing live wiring is covered on a switch box in the storeroom near the emergency exit. This most recently occurred on and preceding August 12, 2022, at 3631 Sullivant Avenue Columbus, OH 43228. A metal junction box housing a light switch has a damaged faceplate which exposes the internal wiring thereby exposing employees to electrical shock and/or electrical fire hazards. Family Dollar Stores Inc. was previously cited under the name Family Dollar Stores of GA, LLC DBA Family Dollar Store # 350 for a Repeat-Serious violation of this occupational safety and health standard 29 CFR 1910.305(b)(2)(i), which was contained in OSHA inspection number 1341910, citation number 1, item number 4 and was affirmed as a final order on February 6, 2019, with respect to a workplace located at 6113 Fairburn Rd., DOUGLASVILLE, GA 30135.
