29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about August 18, 2022, the employer Good Guys Roofing LLC at worksite located at 1501 Pray Blvd. Bldg. D Waterville OH 43566, did not ensure the side rails of one portable 16 feet extension ladder was extended at least three feet above the landing surface. Eight employees were exposed to a fall of approximately 28 feet and greater to the ground below.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 18, 2022, the employer Good Guys Roofing LLC at worksite located at 1501 Pray Blvd. Bldg. D Waterville OH 43566, did not ensure employees were protected from falls while installing tar paper and ice guard on the fourth (roof) level of a new construction residential apartment building. Eight employees were exposed to a fall of approximately 28 feet and greater to the ground below. Good Guys Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1563326, citation number one, item number two and was affirmed as a final order on December 13, 2021, with respect to a workplace located at 607 Pine Valley Ln., Toledo, Ohio 43615.
