29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about August 10, 2023, the employer Good Guys Roofing LLC at worksite located at 595 Sterns Rd. Fostoria OH 44830, did not ensure that employees were protected from head injury hazards by protective hardhats while working directly below employees who were using tools and removing roofing shingles and materials, which was an overhead struck by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 18, 2022, the employer Good Guys Roofing LLC at worksite located at 595 Sterns Rd. Fostoria OH 44830, did not ensure employees were protected from falls while conducting residential roofing construction activities on the third (roof) level of a residential single family home, while exposed to fall hazards of approximately 25 feet and greater to the ground below. Good Guys Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1563326, citation number one, item number two and was affirmed as a final order on December 13, 2021, with respect to a workplace located at 607 Pine Valley Ln., Toledo, OH 43615. Good Guys Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1615770, citation number two, item number one and was affirmed as a final order on October 5, 2022, with respect to a workplace located at 1501 Pray Blvd. Bldg. D, Waterville, OH 43566.
