On or about August 23, 2022, at a work-site located at 960 Duckhollow Cir NE, N. Canton, Ohio 44720; during the installation of roofing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 10.7 feet.

29 CFR 1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about August 23, 2022, during the installation of roofing material at a job site located at 960 Duckhollow Cir NE, N. Canton, Ohio 44720; the employer did not ensure that all employees working on the roof single family residential home were protected from a fall of approximately 10.7 feet by utilizing a fall protection system.
