29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about June 25, 2025, at 6242 Adair Court, Brookpark, OH 44142, the employer failed to ensure employee operating a Metabo HPT Pneumatic Nail Gun, Model number NV 45AB2, wore proper eye protection in that no eye/face protection was worn.   Super Roofing 502, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1780742, citation number 01, item number 001 and was affirmed as a final order on October 31, 2024, with respect to a workplace located at 7044 West 130th Street, Middleburg OH 44130.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 25, 2025, at 6242 Adair Court, Brookpark, OH 44142, the employer failed to ensure employees working on a single family one story home with a 4:12 pitch roof were protected from a fall of approximately 15 feet in that no personal fall protective systems/devices were being utilized.  Super Roofing 502, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1780742, citation number 02, item number 001 and was affirmed as a final order on October 31, 2024, with respect to a workplace located at 7084 West 130th Street, Middleburg Heights OH 44130.  Super Roofing 502, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1616738,  citation number 01, item number 002 and was affirmed as a final order on November 18, 2022, with respect to a workplace located at 960 Duckhollow Circle NE, North Canton, OH 44720.
