29 CFR 1926.102(a)(1):The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about October 9, 2024, at 7084 West 130th Street, Middleburg OH, 44130, the employer failed to ensure employees operating a Bostitch pneumatic nail gun, model number RN46-1 wore proper eye protection in that no eye protection was worn.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: On or about October 9, 2024, during the installation of roofing material at a jobsite located at 7084 West 130th Street, Middleburg OH, 44130, the employer failed to ensure employees working on a single family one story home with a 4:12 pitch roof were protected from a fall of approximately 9 feet in that no personal fall protective systems/devices were being utilized. Super Roofing 502, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 1616738, citation number 01, item number 002 and was affirmed as a final order on November 18, 2022, with respect to a workplace located at 960 Duckhollow Drive NE, North Canton, OH 44720.
