29 CFR 1910.178(l): This standard is applicable to construction employment as defined by 1926.602(d): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  The employer is failing to protect employees from struck-by hazards associated with forklift operation. This was most recently documented on August 31, 2022 at the jobsite located at 18748 Bedford Avenue, Elkhorn, Nebraska. The employee that was operating the forklift was not trained and certified on forklift operation.

This was most recently documented on August 31, 2022 at the jobsite located at 18748 Bedford Avenue, Elkhorn, Nebraska. An employee was using nail guns without eye protection.

This was most recently documented on August 31, 2022 at the jobsite located at 18748 Bedford Avenue, Elkhorn, Nebraska. Two employees that were installing trusses on the top plate of a residential construction building were not using fall protection.

This was most recently documented on August 31, 2022 at the jobsite located at 18748 Bedford Avenue, Elkhorn, Nebraska. The employees were not trained on the recognition of fall hazards and the methods to mitigate these hazards.
