29 CFR  1926.503(b)(1):The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. Employees engaged in residential construction activities at 11877 Ball Parkway, in Papillion, Nebraska 68046,  were exposed to fall hazards in that they were working at heights greater than 6 feet and were not  properly using a personal fall arrest system .  The employer indicated that he has provided his employees with fall protection training, however, the employer indicated that he does not have  written certification that his employees have been trained.
