FCA LLC dba FCA Packaging - On or about 9/1/2022 and times prior thereto: Household-type box fans were used in several areas of the facility. The fans were not listed and labeled for use in industrial setting. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

FCA LLC dba FCA Packaging - On or about 9/1/22, and at times prior thereto, dry chemical portable fire extinguishers located throughout the facility were not visually inspected on a monthly basis by the employer.
