Located in the maintenance facility, the employer did not assure that all portable fire extinguishers were mounted, located, and identified so they were readily accessible without subjecting the employees to injuries. Four fire extinguishers were stored on the floor and were partially blocked by an air compressor hose and cardboard boxes.
