Employees were standing on scaffold platforms attached to the face of cast in-place concrete forms that were not at least 18 inches wide, thereby exposing employees to fall hazards of approximately 10 feet.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access: a) Jobsite located at or near 4224 Baltimore Ave, Philadelphia, PA - On or about September 2, 2022, the employer did not ensure employees were provided a safe means of access to scaffold platforms greater than 2 feet from the point of access. An employee was climbing the face of formwork to access scaffold platforms, thereby exposing the employee to fall hazard of approximately 9 feet.
