29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   a) Anchor Construction, Inc. temporary worksite located at 237 North 17th Street, Philadelphia, PA 19102: The employer did not instruct each employee on identifying working conditions that exposed employees to overhead and trip hazards while engaged in roofing activities.  The employees were not trained on how to recognize, evaluate and mitigate overhead and trip hazards.    Condition noted on or about September 9, 2022.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Anchor Construction, Inc. temporary worksite located at 237 North 17th Street, Philadelphia, PA 19102: The employer did not provide fall protection training to employees engaged in roofing activities on a steep roof that were exposed to falls of approximately 50 feet above the ground level. The employees were not trained on how to recognize, evaluate, and mitigate fall hazards.   Condition noted on or about September 9, 2022.

29 CFR  1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  a) Anchor Construction, Inc. temporary worksite located at 237 North 17th Street, Philadelphia, PA 19102:  The employer did not ensure the work areas and passageways were maintained free of debris during the course of construction.  Roofing materials were removed and disposed on the ground in areas employees accessed as walking spaces, thus exposing employees to trip hazards.  Condition noted on about September 9, 2022.
