29 CFR 1926.20(b)(1): The employer did not initiate and maintain programs necessary to ensure compliance with the Occupational Safety and Health Standards:        Employee(s) engaged in roofing work using ladders, powered tools, fall protection, and etc. at 15320 W. 87th Street, Lenexa, Kansas were exposed to struck by and fall hazards in that the employer had not developed nor implemented safety and health programs that provide construction hazard recognition guidelines to mitigate those hazards.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:       Employee(s) engaged in roofing at a church at 15320 W. 87th Street, Lenexa, Kansas were exposed to struck by, crushing, and fall hazards in that employees were not instructed in the recognition of construction hazards and the methods to mitigate those hazards.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  Roofing employees engaged in removing roofing materials from the roof located at 15320 W. 87th Street, Lenexa, Kansas were exposed to a fall hazard of approximately 40 feet due to no fall protection system being used.  The M&A Roofing KC LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1542961, citation number 1, item number 1 and was affirmed as a final order on December 30, 2021, with respect to a workplace located at 5423 NW Platte Purchase Dr., KCMO.  The M&A Roofing KC LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1542911, citation number 1, item number 2 and was affirmed as a final order on December 30, 2021, with respect to a workplace located at 5423 N Platte Purchase Dr., KCMO.
