29 CFR 1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: On or about September 16 2022, the employer did not store carbon dioxide cylinders in accordance with the follow paragraphs of the Compressed Gas Association Pamphlet P-1-1965: Two blue carbon dioxide cylinders did not have protective caps present on them (in accordance with paragraph 3.1.11 and 3.3.8).

29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition: On and before September 16, 2022, a dock leveling/plate was not inspected regularly and as necessary to maintain the device in a safe condition. The device did not fully fold out to ensure contact with the trailer and employees would be exposed to nip point hazards when attempting to assist the device during this process.
