On the roof of a residence, at a temporary worksite located at 5959 Ft. Caroline Road in Jacksonville, Florida: On September 15, 2022, the employer exposed employees to the hazard of being struck in the eye by debris and nails, in that safety eyewear was not utilized by employees, while using pneumatic nail guns to install roofing shingles.

Caroline Road in Jacksonville, Florida: On September 15, 2022, the employer exposed employees to fall hazards ranging from 6 to 19-foot, in that fall protection was not utilized by employees while performing roofing work to install roofing shingles.
