29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On a 5:12 pitched roof of a residence, at a temporary worksite located at 5959 Ft. Caroline Road, building 59, in Jacksonville, Florida: On or about October 14, 2022, the employer exposed employees to fall hazards ranging from  11.2 to 17.7-foot, in that fall protection was not utilized by employees while performing roofing work to install roofing shingles.

29 CFR  1926.405(g)(1)(ii): Attachment plugs for cords. If used as permitted in paragraphs (g)(1)(i)(C), (g)(1)(i)(F), or (g)(1)(i)(H) of this section, the flexible cord shall be equipped with an attachment plug and shall be energized from a receptacle outlet.  a. At a multifamily residential, located at 5959 Fort Caroline Road, building 59, Jacksonville, FL 32277: On or about October 14, 2022, the employer allowed employees to use an extension cord as the main power source that was modified at the plug end and was used other than as tested for UL rating while was hard wire to a receptacle outlet, exposing employees to burns and/or electrocution.
