At ground level, at a temporary worksite located at 8515 Hampton Ridge Boulevard in Jacksonville, Florida: On September 19, 2022, the employer exposed two employees to head injury from overhead hazards, such as falling roofing debris, in that protective helmets (hard hats) were not utilized by employees who were working beneath employees on the roof while performing debris clean-up around the perimeter of the roof.
