29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a. At ground level, at a temporary worksite located at 8515 Hampton Ridge Boulevard in Jacksonville, Florida: On September 19, 2022, the employer exposed two employees to head injury from overhead hazards, such as falling roofing debris, in that protective helmets (hard hats) were not utilized by employees who were working beneath employees on the roof while performing debris clean-up around the perimeter of the roof.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a. On a 10:12 pitched roof of a commercial building, at a temporary worksite located at 8515 Hampton Ridge Boulevard in Jacksonville, Florida: On September 19, 2022, the employer exposed employees to 8-foot, 6-inch fall hazards, in that fall protection was not utilized by employees, while performing roofing work.  b. On a 10:12 pitched roof of a commercial building, at a temporary worksite located at 8515 Hampton Ridge Boulevard in Jacksonville, Florida: On September 19, 2022, the employer exposed employees to 15-foot fall hazards, in that fall protection was not utilized by employees, while performing roofing work.
