OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from crushing hazards: (a) 6087 Screaming Eagle Blvd, Fort Campbell, KY: On or about 09/15/2022, and at times prior, the employer exposed employees to crushing injuries or death from mower rollover on grades sloped at angles that exceed the manufacturer's recommended limit of 15 degrees, in that, employees were operating SCAG Turf-Tiger II Zero Turn Mowers in culverts with a grade of 23.5 degrees. (b) 6087 Screaming Eagle Blvd, Fort Campbell, KY: On or about 09/15/2022, and at times prior, the employer exposed employees to crushing injuries or death from mower rollover on grades sloped at angles that exceed the manufacturer's recommended limit of 15 degrees, in that, employees were operating SCAG Turf-Tiger II Zero Turn Mowers in culverts with a grade of 17 degrees. (c) 7404 McAuliffe Way, Fort Campbell, KY: On or about 09/28/2022, and at times prior, the employer exposed employees to crushing injuries or death from mower rollover on grades sloped at angles that exceed the manufacturer's recommended limit of 15 degrees, in that, employees were operating SCAG Turf-Tiger II Zero Turn Mowers on a hill with a grade of 27.1 degrees.

(b) Maintenance/Admin building 6468 53rd Street, Fort Campbell, KY: On or about 9/15/2022, and at times prior, the employer exposed employees to lacerations and serious eye injury, in that employees were operating SCAG Turf Tiger II zero radius mowers performing mowing operations with shoot guards that were tied in an up position. (c) Range 22 Angels Rd., Fort Campbell, KY: On or about 9/16/2022, and at times prior, the employer exposed employees to lacerations and serious eye injury, in that employees were operating SCAG Turf Tiger II zero radius mowers performing mowing operations without shoot guards.

(b) Bldg. 6468 - at 53rd St., Fort Campbell, KY: On or about 9/15/2022, and at times prior, the employer exposed employees to caught-in / pinch hazards, in that the belt/spindle guards were removed from the mower decks of eight SCAG Turf Tiger II zero turn mowers exposing the drive belts and pulleys while employees were operating the mowers. (c) Bldg. 6468 - at 53rd St., Fort Campbell, KY: On or about 9/15/2022, and at times prior, the employer exposed employees to caught-in / pinch hazards, in that the belt/spindle guards were removed from the mower decks of a Land Pride All-Flex Mower (AFM4216) exposing the drive belts and pulleys while employee was operating the mowers. (d) Range 22 - Angels Rd., Fort Campbell, KY: On or about 9/16/2022, and at times prior, the employer exposed employee to caught-in / pinch hazards, in that the belt/spindle guards were removed from the mower deck of a SCAG Turf Tiger II zero turn mower exposing the drive belts and pulleys while employee were operating the mower.
