29 CFR 1910.29(f)(4): Openings in stair rails. The employer did not ensure that no opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension. On or about September 21, 2022, Patterson Rig #289, employees were exposed to fall hazards because employer did not ensure that no opening in a stair rail system exceeded 19 inches in the following instances: a) 1-Staircase on Trailer #020131560 b) 2-Staircases on Trailer # MH0757 c) 3-Staircases on Trailer #MH0930 d) 2-Staircases on Trailer #MH964-R e) 2-Staircases in front of Company Representative Trailer. This condition exposed employees to fall hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: On or about September 21, 2022, at Patterson Rig # 289 rig, a strain relief on a black portable power cable (marked 480V) was pulled away from plug on Trailer #MH7-0237R. This exposed employees to electrical shock hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): On or about, September 21, 2022, at the Precision Rig #289 Rig, a damaged extension cord (pinched/crushed outer jacket) was being utilized from the water pump to the water wagon trailer. This condition exposed employees at the rig site to electrical hazards such as electrical shock and/or burns.
