29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:    14625 Potranco Road, San Antonio, TX: On or about October 12, 2022, and at times prior thereto, workers were exposed to fall hazards while using an aerial lift to lower themselves from the top of a roof without wearing a fall harness.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   14625 Potranco Road, San Antonio, TX: On or about October 12, 2022, and at times prior thereto, workers were exposed to fall hazards while installing roofing panels to the roof of a residential new build without the use and wear of a personal fall restraint system.
